Went to equip the follower Kor'vas Bloodthorn after I got her first equipment slot open with a piece of equipment, the lucky Doodad. It instead took the item but did no upgrade.

This was confirmed not in effect as the lucky doodad would change the % success chance after placing the same follower on a mission, and putting her on the same mission I almost sent her on before equipping her yielded the exact same result, so it consumed the gear but gave none of the benefit (and her equipment is still empty, it behaves as such if I query the slot)

I am unaware if this is affecting all followers or just Bloodthorn. But without equipment many high grade missions will be very unlikely to work.

(She was not out on a mission when I attempted this, I sent her out anyways to get the mission rolling after it consumed the item.)
